In consideration of being granted permission to participate in a photography session with JRD Dance Media, I, the undersigned participant, hereby acknowledge and accept the terms and conditions outlined below:
Assumption of Risks: I understand and acknowledge that participation in a photography session involves inherent risks and dangers, including, but not limited to, the risk of physical or emotional harm, injury, or even death. These risks may arise due to the negligence of JRD Dance Media or third parties involved.
Voluntary Participation: I confirm that my participation in the photography session is voluntary and that I have voluntarily chosen to engage in this activity. I am responsible for my own safety and well-being throughout the session.
Release and Indemnity: On behalf of myself and my representatives, including heirs, executors, administrators, assigns, and agents, I hereby release, discharge, and hold harmless JRD Dance Media, its agents, employees, officers, and assigns from any and all claims, demands, liabilities, actions, or causes of action arising out of or in connection with my participation in the photography session. This release applies to claims whether known or unknown, anticipated or unanticipated, in law or in equity.
Ownership and Use of Photographs: I acknowledge that JRD Dance Media retains full ownership and copyright of all photographs taken during the photography session. I grant JRD Dance Media the unrestricted right to use, reproduce, and publish these photographs for marketing, promotional, and social media purposes without any compensation to me.
Compliance with Instructions and Rules: I agree to comply with all instructions provided by JRD Dance Media, its agents, employees, and assigns during the photography session. I also agree to adhere to all rules and regulations set forth by JRD Dance Media for the duration of the session.
Age Certification: I certify that I am either 18 years of age or older. If I am under 18 years of age, I affirm that my parent or legal guardian has provided their consent by signing this waiver on my behalf.
Use of Images on Social Media: I acknowledge that images or video clips featuring myself or my children may appear on JRD Dance Media's social media accounts, considering my association as a YouTube content creator.
By signing this Participant Waiver and Release, I confirm that I have read, understood, and agreed to all its provisions. I acknowledge the importance of seeking independent legal advice if I do not fully comprehend any aspect of this waiver before signing it. My signature below signifies my voluntary acceptance of all the terms and conditions outlined herein.
